Terms and Conditions & Privacy Policy

Terms and Conditions


By booking an appointment, attending a consultation, or receiving treatment at Morteza Jalali Medical Aesthetics (“Clinic”, “we”, “our”), you (the “Patient”) agree to these Terms and Conditions. All services are provided at our location in Chiswick, West London. You also acknowledge that our separate Privacy Policy applies.


General
1.1 These Terms form a binding agreement between the Patient and the Clinic.
1.2 They cover all consultations, treatments, services, and products provided.
1.3 All appointments, consultations and treatments must take place at the location specified by the Clinic at the time of booking.


Appointments and Deposits
2.1 Medical consultations cost £75 and are payable at the time of booking. Fees are non-refundable except in the event of clinic cancellation or exceptional documented emergencies (e.g., hospitalisation, death of immediate family member, or serious illness evidenced by medical documentation). Consultation fees may be redeemed against treatment undertaken within three months of the consultation date. Model treatments follow clause 17.5.
2.2 A 50% deposit is required to secure treatment appointments. This deposit is non-refundable except where the Clinic cancels the appointment or in the event of exceptional documented emergencies as defined above.
2.3 Model treatments require a £100 deposit. Full model treatment terms are set out in clause 17.


Cancellation and Rescheduling
3.1 Patients must provide at least 72 hours’ notice to cancel or reschedule.
3.2 Failure to provide notice or attend will result in forfeiture of the deposit.
3.3 Arriving more than 15 minutes late may result in cancellation or rescheduling at the Clinic’s discretion. Deposits may be applied to rescheduled appointments at the Clinic’s discretion.
3.4 Exceptional circumstances such as serious illness, injury, or bereavement may allow fees to be waived if documented.


Eligibility
4.1 Treatments are only available to patients aged 18 or over. The Clinic may request valid photographic identification to verify age where necessary.
4.2 Treatments are not provided to individuals who are pregnant, breastfeeding, or medically unsuitable.
4.3 Patients must provide complete and accurate medical information, including allergies and medications.
4.4 The clinician may request further medical information, investigations, or confirmation from a patient’s GP or treating specialist where necessary to ensure safe treatment.
4.5 Informed consent must be given for all treatments, covering risks, side effects, and use of clinical photographs.


Medical Consultation and Assessment
5.1 A consultation is required for all new patients or new courses of treatment.
5.2 The clinician will discuss suitable treatments, realistic outcomes, potential risks, and costs.
5.3 Treatment will only proceed if deemed clinically safe and appropriate.
5.4 Risks and realistic outcomes are documented as part of the consent process.

5.5 The Clinic reserves the right to modify or decline a treatment plan at any time if new medical information arises or if the clinician determines that proceeding would not be in the patient’s best clinical interests.


Treatments Provided
6.1 Treatments include, but are not limited to:
(a) Botulinum toxin injections to relax muscles and reduce lines and wrinkles.
(b) Profhilo® regenerative hyaluronic acid treatment for hydration and firmness.
(c) Polynucleotides for regenerative treatment to improve hydration and skin quality.
6.2 Multiple sessions may be required for optimal results.
6.3 Treatment outcomes and duration of effect vary between individuals, and no specific outcome or duration of effect is guaranteed.


Risks and Complications
7.1 Common side effects include swelling, redness, bruising, tenderness, or temporary asymmetry.
7.2 Rare complications include allergic reactions or uneven responses.
7.3 All risks are discussed during the consultation and documented in consent forms.

7.4 Patients must follow all aftercare instructions provided by the clinician. Failure to follow aftercare advice may increase the risk of complications and may affect the Clinic’s ability to provide corrective treatment.


Review Appointments for Botulinum Toxin
8.1 Review appointments are offered approximately two weeks after treatment.
8.2 One complimentary adjustment may be offered at the clinician’s discretion if clinically necessary and within the recommended timeframe.
8.3 Patients must book review appointments promptly; failure to attend may forfeit the adjustment.


Fees and Payment
9.1 A 50% deposit secures treatment appointments, deducted from the total cost. Remaining balance is due at the time of treatment or final treatment session.
9.2 Payment methods: debit card, credit card, or bank transfer.
9.3 Prices may change, but confirmed bookings honour the quoted price.
9.4 Consultation fees follow clause 2.1.


Refund Policy
10.1 Treatment fees are non-refundable solely because outcomes differ from expectations.
10.2 Refunds may be considered only in cases of documented clinical complications and remain at the sole discretion of the Clinic.
10.3 Patients acknowledge that results vary due to biological differences.


Photographs and Medical Records
11.1 Clinical photographs are taken before and after treatment for records.
11.2 With written consent, photographs may be used for education or marketing. Withdrawal of consent will not affect materials already published or used prior to withdrawal.
11.3 Records are stored securely in line with UK medical regulations.


Right to Refuse Treatment
12.1 The Clinic may decline or postpone treatment if:
(a) Treatment is not clinically appropriate.
(b) Expectations are unrealistic.
(c) There are medical or psychological concerns.
(d) The patient behaves inappropriately or refuses to follow safety instructions.

12.2 All refusals or postponements will be documented and based on professional clinical judgement and in accordance with UK medical standards.


Complaints
13.1 Complaints must be submitted in writing to practice@mortezajalali.com.
13.2 The Clinic will acknowledge receipt within five working days.
13.3 Resolution will be sought within 28 days of acknowledgment.


Limitation of Liability
14.1 The Clinic is not liable for indirect or consequential losses, including lost earnings or travel expenses.
14.2 Liability for death, personal injury caused by negligence, fraud, or other non-excludable liability is not limited.
14.3 The Clinic is not liable for outcomes differing from expectations where treatment was provided in accordance with medical guidance, informed consent, and the aftercare obligations set out in clause 7.4.

14.4 Patients are responsible for providing accurate medical information. The Clinic is not liable for adverse effects resulting from the misrepresentation or omission of relevant medical history.

14.5 The Clinic is not liable for failure or delay in performing its obligations where such failure results from events beyond its reasonable control, including but not limited to illness, natural disasters, power failures, or government restrictions.


Governing Law
15.1 These Terms are governed by the laws of England and Wales.
15.2 Disputes fall under the exclusive jurisdiction of the courts of England and Wales.


Referral Programme
16.1 Patients may refer friends or family for treatments.
16.2 Referrals must identify the referring patient at booking.
16.3 Both the referring and new patient will receive £30 off their next treatment if conditions are met.
16.4 Discounts apply to new patients only. The Clinic may refuse if conditions are not met.
16.5 Participation constitutes agreement to these Terms, including deposits and cancellations.
16.6 Referral discounts do not affect clinical recommendations.


Cosmetic Model Treatments
17.1 Applications must include name, date of birth, phone number, three facial photos, and treatment area.
17.2 Applicants must be 18+, not pregnant or breastfeeding.
17.3 Consent is given for photographs and video for education, promotion, or social media. Withdrawal of consent will not affect materials already published or used in marketing prior to withdrawal.
17.4 Acceptance is at the sole discretion of the Clinical Director, Dr Morteza Jalali, based on clinical suitability.
17.5 £100 deposit is required, deducted from the total cost. No separate medical consultation fee applies. Deposits are non-refundable for no-shows, cancellations, or appointment changes by the patient, unless an exceptional documented emergency occurs.

17.6 Results of treatment are not guaranteed. Outcomes may vary due to individual biological factors.

17.7 Successful applicants receive a personalised assessment with recommended treatments and costs.

Consent for Marketing and Clinical Photographs

7.2 Consent may be withdrawn at any time in writing by contacting privacy@mortezajalali.com. Withdrawal will not affect previously published materials, and all withdrawals will be documented.
